Therapy Schedule

The short-term rehab program is intense and accelerated, and also tailored to the individual. It helps both inpatients and outpatients build strength and independence to allow individuals to achieve their maximum level of function before returning home. Your therapy schedule may include two or more individualized therapy sessions per day. During your stay, the therapists will have you practice, perform, and perfect all the skills you need before being discharged.

Following the course of care, each patient is evaluated to determine their ability to continue living alone or with loved ones. Most rehabilitation patients return to their own homes or a Supported Living site following recovery.

Long-Term Care

For those unable to live independently at home we offer medically supervised long-term care. Typically, people in need of this type of service require long-term, 24-hour care by a team of skilled clinicians including nurses, physical therapists, and occupational therapists. The most common needs of residents at skilled nursing centers include daily medical management, and assistance with activities of daily living.

For patients requiring an extended placement, we honor the opportunity to help residents and families create a home for their loved one at our community. Our Restorative team works closely with the rehabilitation department to design individual programs for walking, strengthening, range of motion, pain management, and group exercises. Our long-term care program offers specialized nursing care, diverse recreational activities and supportive social services catered to the resident’s individual needs. In addition, our physicians will oversee your loved one’s medical care along with our interdisciplinary team of licensed professionals.

We encourage family to become involved with us in creating a living space that has familiar items from home. Whatever your needs, we are here to help. For some residents, we are able to restore them to a level of function which enables them to return home. For those who cannot return home, we are committed to providing a warm and caring home and feel honored they chose to make our community their home.


Managing medications

Our pharmacists work with our skilled nursing team to manage each guest’s medications to ensure the proper dosage is prepared for the guest’s existing medications, as well as, any new medications. We encourage all guests to complete a Medication Record prior to checking in. To facilitate the guests’ continuation of care, upon discharge, we inform the guest’s primary care physician of any new medications.
